1. HERE'S HOW TO ENTER: Go online to www.patrickcudahy.com and click on the Grand Geneva logo located on the Promotions page, or mail a plain piece of 3 x 5 inch paper hand printed with your name, complete street address, and daytime phone number to ” Patrick Cudahy Incorporated “Grand Geneva Getaway,” c/o Esrock Partners, 14450 S. 94th Avenue, Orland Park, IL 60462. By entering this Contest each entrant agrees that the Sponsor for lost, late, illegible, misdirected or postage-due entries assumes no responsibility. All entries must be postmarked on or by 7/29/05 and received on or by 8/1/05, to be eligible for the drawing. The drawing date to determine winner will be on or around August 5, 2005.
2. PRIZE: Prize: One winner. Three (3) days / two (2) nights stay at the Grand Geneva or Timber Ridge Lodge Waterpark located in Lake Geneva WI. Prize includes lodging for a family of four. Four tickets to the Timber Ridge Lodge Waterpark. Additional Grand Geneva Fun money in the amount of $200 will also be included to use at the Spa, Golf Course, shopping, restaurants and more. Excludes Saturday and Holidays. Approximate retail value $800. All transportation costs, meals, tips and other expenses associated with accepting the prize, other than those specifically included, will be the sole responsibility of the winner.
3. DRAWING: The prize winner will be selected in a random drawing from among all eligible entries received. Drawing will be held on or about 8/5/05 and will be conducted by Esrock Partners an independent judging organization. All decisions of the judges shall be final. Odds of winning depend upon the number of eligible entries received. All prizes will be awarded. The winner will be notified via the Patrick Cudahy website www.patrickcudahy.com and a daytime phone number that is provided.
4. ELIGIBILITY: You must be a legal resident of the U.S. and live in Illinois, Wisconsin, Indiana or Iowa, and be 18 years and older, to be eligible. Employees of Patrick Cudahy Incorporated, Esrock Partners, Grand Geneva Resort and participating retail outlets, their parent companies, subsidiaries, affiliates, and advertising, promotional agencies, employees (collectively, “Sponsor Parties”), and immediate family members of the aforementioned are not eligible to win. Void where prohibited or restricted by law.
5. CONDITIONS OF PARTICIPATION: The winner and their guests must sign and return an Affidavit of Eligibility, Liability and Publicity Release form. In the event the affidavit and release is not returned within 14 days of request, the prize will be forfeited. Any prize notification returned to Sponsor as undeliverable will result in the forfeiture of the prize. All winners by participation in this Sweepstakes, agree to release Sponsor Parties from any and all liability, claims or actions of any kind whatsoever for injuries, damages or losses to persons and property which may be sustained in conjunction with the receipt, ownership or use of the prize or while preparing for, participating in, and/or traveling to any prize-related activity. It is the winner's financial responsibility to travel to Lake Geneva to the Grand Geneva or Timber Ridge Lodge. All transportation costs, meals, tips and other expenses associated with accepting the prize, other than those specifically included, will be the sole responsibility of the winner. Parent/legal guardian must accompany minors, who are guests of the winner. Prize will be forfeited if winner is unable to travel on designated dates. Excludes Saturdays and Holidays, Trip good for 1 year. Winner, upon acceptance of the prize, consents to use his/her name, voice, picture, and/or other likeness without compensation for promotional purposes as determined by Sponsor Parties. All taxes to which the prize may be subject are the sole responsibility of the winners. Winners will receive a Form 1099. Prizes cannot be redeemed for cash value and are non-transferable. By participating in this promotion, you agree to be bound by these Official Rules and the decisions of the judges which shall be final and binding. You are not a winner until you have complied with these Official Rules.
6. VERIFICATION: All online submissions and sweepstakes entry forms become the property of Patrick Cudahy Incorporated and will not be returned. All entry forms subject to verification at the sole discretion of Patrick Cudahy Incorporated. Anti-fraud detection devices will be used. Entry forms will be void if they fail to pass anti-fraud detection measures or if found to be photocopied, reproduced, altered, forged, defaced, tampered with, mutilated, stolen, un-issued, unauthorized, unreadable, partially printed, incompletely printed, smudged, crushed, or illegible in any way, or if they fail to contain special markings, inks and/or security codes used by Patrick Cudahy Incorporated in this promotion, or if defective or if they contain printing or other mechanical errors or are printed or produced in error. No more than the stated number of prizes in these official rules will be awarded.
